Material Failures and Malfunctions: Factors, other than human error, that caused or contributed to the accident and or resulted in a loss of property, income, or personnel. Below are some common equipment failures causes and I also serve as a checklist for loss prevention.

1. Overheated/Burned/Melted: Excessive heat caused material or equipment to fail or malfunction.

2. Froze (temperature): Excessive cold caused material/equipment to fail or malfunction.

3. Obstructed/Pinched/Clogged: Function or material or equipment was hindered or completely cut off by an obstacle.

4. Vibrated: Side-to-side or forward-and-back movement of material or equipment caused it to fail or malfunction.

5. Rubbed/Worn/Frayed: Friction-producing movement was applied to material or equipment to such an extent that it failed or malfunctioned.

6. Corroded/Rusted/Pitted: Gradual wearing away (usually by chemical action) of material or equipment to such an extent that it failed or malfunctioned.

7. Overpressured/Burst: Steady or abrupt force was applied over the surface of the material or equipment to such an extent that it failed or malfunctioned.

8. Pulled/Stretched: Steady or abrupt force applied to material or equipment caused it to move toward the force, in whole or in part, to such an extent that it failed or malfunctioned.

9. Twisted/Torqued: Steady or abrupt application of twisting forces caused material to fail or malfunction.

10. Compressed/Punctured: Steady or abrupt application of force that presses/.impacts material or equipment causing it to fail or malfunction.

11. Bent/Warped: Changing material or equipment from an original straight, level, or even condition through the application of force to such an extent that it failed or malfunctioned.

12. Sheared/Cut: Failure or malfunction was caused by steady or abrupt force applied to material, resulting in a break with two parts sliding parallel to each other in different directions.
